YMCAs are pioneering an innovative Medical Membership model where three services are bundled together:
- a YMCA membership
- participation in an evidence-based program and
- care coordination.
With a physician referral, individuals and families are referred to the YMCA as a result of a health diagnosis, such as pre-diabetes, high blood pressure, adult and child weight management, cancer or risk of falls due to age or illness.
